Our Team

Sarah Kowalczk, DPT, CLT

Sarah is a certified lymphedema therapist and doctor of physical therapy, serving as program director for ReVital Cancer Rehabilitation for Alaska. ReVital was born to Select Medical gathering the greatest minds in cancer research, rehab and intervention with the goal of allowing individuals to save their quality of life during and after cancer. The mission of ReVital is to optimize patients' quality of life, no matter what phase of their cancer journey they may be on. Sarah is working in the outpatient setting with a special focus in cancer rehabilitation and functional strength and endurance training. Also a Graston technique specialist/ certified dry needling physical therapist basing practice around the mind body connection and the importance of promoting active lifestyles for all individuals.

Amy Sauck, P.T., DPT, COMT

Amy specializes in treatment of chronic pain, work-related injuries and return to work. Amy has a unique interest in the relationship between functional mobility, injury prevention and psychosomatic factors. Amy is diligent with her care plan creation and work tirelessly to return her patients to work, sport and the active lifestyle required of Alaskans, no matter their age or injury. Amy's willingness to strive for growth is rooted in optimizing care for her patients. Amy's specialties set her apart from many other therapists as she has curated an extensive educational and resource toolkit that allows her patients to have the self-efficacy to take their care into their own hands for life long change.

Jennifer Goetz, OTR/L

Jennifer is an occupational therapist who recently joined the Select Physical Therapy team. She received a master's degree in occupational therapy from the University of Mary in 2010. She has treated for 12 years in a variety of settings including outpatient, acute care, acute rehabilitation, nursing home and home health. She is passionate and has focused ongoing training and certification in treating those who have had a neurological event impacting function including stroke, spinal cord injuries, Parkinson's and brain injuries.

Shelby Blum, P.T., DPT

Shelby specializes in vestibular and concussion rehabilitation and is a trained ReVital Cancer Rehabilitation specialist. Shelby brings a holistic approach to care with particular interest multi-system involvement, leading to her specialty certifications. Shelby is also certified in the Strength After Breast Cancer Program, providing patients acute and long-term intervention strategies to help build strength and self-efficacy throughout the cancer continuum. Shelby takes pride and attention to making patients feel like they can overcome whatever obstacle they are facing. Shelby takes the time to not only get to know patients, but their families and favorite activities to help build goals to optimize quality of life and return to what they love.

Kayla Mathiowetz, M.S., CCC-SLP

Kayla is a speech-language pathologist and has been a member of Select Kids Pediatric Therapy since May 2022. Kayla received a master's degree in communication disorders from Minnesota State University in 2016 and has worked in outpatient pediatrics and adults, inpatient, skilled nursing facilities and home health. Kayla was a traveling speech therapist for many years before making Alaska her home. Kayla specializes in treating children with speech sound disorders, stuttering, language disorders, voice disorders and literacy difficulties. She has a passion for helping children find their voice. She loves hiking, camping and traveling.

Hand Therapy

Our certified hand therapists (CHT) provide comprehensive and industry-leading treatment for hand, wrist, arm, elbow and shoulder injuries or conditions. Our team will work with you to create an individualized program to return you to work, life and athletics as soon as possible.

Pediatric Therapy

We offer multi-disciplinary pediatric therapy services – physical, occupational, speech and applied behavioral analysis (ABA) therapy – to help children reach developmental milestones and their full potential. Therapy methods are typically provided in one-on-one sessions, using play-based activities to gain insight into how a child moves, thinks and expresses themselves. Small group therapy also can be helpful for some children to build social skills and communication. Additional specialized programs, such as feeding therapy, sensory/emotional regulation, toe-walking and torticollis, are also available at select locations. We are committed to providing an exceptional experience that enhances each child's participation in desired family, community and school activities.
