Our Team

Ondre Smith, P.T., DPT, Center Manager

Ondre brings a high level of energy and personal to the patients he treats. His bright colored outfits match his outgoing, yet caring personality. Ondre attended Regis University and earned a doctorate of physical therapy at University of Colorado University in 2020. Ondre enjoys manual therapy, treating patients returning to sports, pre- and post-operative rehabilitation, low back, neck and headache rehabilitation. He is also certified in dry needling

Paige Schreiber, P.T., DPT

Paige treats patients with compassion, understanding and an open mind. She strives to help improve quality of life through physical therapy and enjoys treating a wide range of patients and advocates for overall health and wellness. Her treatment skills include vestibular and concussion care and dry needling. Paige earned a doctorate of physical therapy at University of Wisconsin - Milwaukee, "Go Pack Go". Paige has been with Select since 2021.

Paula Foglia, P.T., DPT, CSCS

Paula has been a member of Select Physical Therapy since 2022. She has worked primarily in the outpatient setting, specializing in orthopedic rehabilitation and sports medicine. Paula completed her undergraduate education doctorate of physical therapy from University of Florida. She also has training in dry needling and a certification in Graston technique. Paula is skilled as pelvic health therapist. She is a true sports fan and fitness enthusiast. She enjoys strength training, yoga, skiing, hiking and cheering on her Florida Gators, Miami Heat and Miami Dolphins.

Sung Yi, P.T., DPT

Sung is a deeply committed advocate for strengthening diverse communities' health, dignity and overall livelihood by providing culturally appropriate care. He received his doctorate of physical therapy from Regis University in 2023. He enjoys treating a wide variety of patients and has his LSVT BIG certification. Sung enjoys spending time with his family and two French Bulldogs. He loves to try new foods and is always looking for great places to eat. He is a native of the Pacific Northwest and loves to support his hometown Mariners and Seahawks.

Alexis Leavers, P.T., DPT

Alexis is a dedicated physical therapist from Northeast Ohio with a passion for movement and performance. She earned her bachelor's degree in health promotion, nutrition and exercise science from Ohio State University before pursuing her doctorate in physical therapy at the University of Colorado. Lexi uses her certified strength and conditioning specialist certification and continuing education to help her patients return to their prior level of function.

Mary Gerken, P.T., DPT,

Born and raised in Texas, Mary moved to Colorado in spring 2026. She brings a high level of energy and personality to the patients she treats. She is passionate about helping them set and achieve physical goals and work toward pain relief.

Mary loves the outdoors and being active - she is often found playing pickleball, hiking or skiing. She also loves to travel and experience new places; her favorite place is at home in Texas with her family.

Success Stories

Martin Clark

Martin Clark lived a fast, athletic life as a downhill ski racer in high school, college and recreationally. Overtime, the "wear and tear" caught up with him. Martin was experiencing stiffness, instability, muscle atrophy and pain that worsened after activity. Nothing was helping and Martin's ability to participate in what he loves began to shrink. Martin came to Select Physical Therapy where he worked with Physical Therapist Paula Foglia. There, Paula designed a custom treatment program to address Martin's goals of getting back to pain-free activities. "In the beginning, I had no expectations," Martin said. "But, I wanted to enjoy my life more. So, I worked with Paula and did my stretches and mobility work." Within a few visits, he could see improvement. This incentivized him to continue. "I am able to hike, walk the dog, bike and work out again," he said. "Even stairs don't intimidate me anymore."
