Pelvic health physical therapy provides care and relief to men and women who experience pelvic floor dysfunction. Our Team

Dan Fog, DPT, OCS

Dan has been practicing outpatient physical therapy since 2005. During this time he has had the opportunity to help individuals of all ages stay active and healthy. He believes the purpose of physical therapy is to enable his patients to live their optimal lives. He focuses on treating the entire person, diving into the cause of an injury, not just treating the symptoms. Dan incorporates a mix of hands-on treatments with a emphasis on progressive functional exercise. Dan graduated from Luther College in 2003 and received his doctorate in physical therapy from the University of Iowa in 2005. He became a board certified orthopedic specialist in 2018. He is certified in Graston augmented soft tissue and dry needling.

Angela Maples, PTA

Angela graduated from St. Ambrose University in 2017 with a bachelor's degree in exercise science, and in 2020 graduated from Kirkwood Community College with an associate's degree to become a physical therapist assistant. Angela has experience working in Outpatient/Inpatient clinical settings working with numerous patients with varying diagnosis. She has a special interest within orthopedics and geriatrics. She joined the Select Physical Therapy Team in 2021.

Anna Chiri, P.T., DPT

Anna graduated from the University of Puget Sound with a doctorate in physical therapy in May 2023. Prior to physical therapy, Anna was a professional dancer, choreographer and certified barre instructor in the Seattle area. She has special interests in aquatic therapy, pelvic health and pain science. Anna is passionate about empowering others with movement in order to optimize their daily function and improve their quality of life.

Stacy Almanza, P.T., DPT, COMT

Stacy has been a member of Select since 2005, with 20 years of clinical experience. She has worked in the outpatient setting. Stacy completed her undergraduate education at the University of Northern Iowa and doctorate of physical therapy from Northwestern University. She specializes in the treatment of post-operative rehab, sports medicine, low back rehabilitation, instrument-assisted soft tissue mobilization, pelvic health and COVID-19 recovery and reconditioning. Additionally, she has certification in cancer care.
